Who is TPQVO?
TPQVO was created in 1998 by merging the
Chattanooga-Hamilton County Medical Society and Knoxville Academy of Medicine
Central Verification Services into one statewide program. TPQVO's mission
is to provide innovative, practical, and top-quality credentials verification
services that will benefit both physicians and healthcare entities by
streamlining a cumbersome paper shuffling task into a quality centralized
solution that saves time and money.
We accomplish the goal of streamlining by:

using a standard initial and
recredentialing application for each region;

pre-populating recredentialing
applications with data in our credentialing database to shorten the form
filling process;

standardizing the recredentialing cycle
among TPQVO client healthcare organizations by specialty so that all
healthcare professionals of the same specialty or licensure (Allied
Health) undergo recredentialing at the same point every two years; and
-
collecting credentialing related
documents such as renewed licenses, DEA registrations and insurance
policies at the same time for all clients with which the professional is
affiliated.

Reappointment or recredentialing forms are not available on this
website. If you need a new reappointment or recredentialing form,
you will need to contact TPQVO directly for a copy.
In Tennessee, most participating healthcare
organization clients have agreed upon the use of one standard
application and recredentialing form. Depending upon your region,
you will select the appropriate form available for filling and printing
using Adobe® Acrobat® Reader™. (This program is available for free
download by
clicking here.) Other healthcare organization clients may use
their own forms or the applicable mandated state credentialing form.
Confidentiality and Privacy
TPQVO maintains credentialing information
under the strictest confidentiality policies. Once we receive your
information, we only release it to those healthcare organizations you
authorize in writing.
